Claim Reporting

Members can now report work injury claims 24 hours a day, 7 days a week by either calling the toll-free reporting hot-line at 1-866-840- 0210 or by filling out the First Report of Injury Form and email form to FROI@mrm-llc.com .

This will help in several ways, including:

• Reduction of paperwork - 
• Reduced claim costs
• Timeliness of benefit delivery

When a worker injury is reported to you, simply call 1-866-840-0210.  Please have the following information ready:

• Injured worker's: Name; Address; Telephone Number; Social Security Number; Date of Hire and Date of Birth
• Date of Injury and Description of Injury
• Name and Address of Medical Provider
• Wage Information
• Ten-Digit Alabama Employer Unemployment Compensation Number
